REPORTING IRREGULARITIES
The form is destined for anonymous reporting irregularities in
Krakchemia S.A. in Kraków („The Company”), implemented in accordance with the procedure of anonymous reporting the irregularities in Krakchemia S.A.
This form is destined for the people employed in the Company on the basis of a job contract, but also on the basis of the contract of mandate, as well a contract of commission, or other contract of similar character („The Reporting Person”) and serves for anonymous reporting to the Company of any identified, undesirable or illegal activities, including the breach of regulations, procedures, ethical standards, in particular, concerning the following:
- The organisation of work,
- Non-observance of internal procedures,
- communication,
- embezzlement,
- mobbing,
- harassment,
- discrimination.
RODO information clause:
1. The administrator of personal data of The Reporting Person is the COmpany, i.e. Krakchemia S.A. with a seat in Kraków, ul. Powstania Listopadowego 14, 30-298 Kraków („The Administrator”).
2. The Administrator informs that:
1.1. Providing personal data of The Reporting Person is free, reporting may be executed anonymously;
1.2. The personal data provided by The Reporting Person will be processed with the aim connected with the executed reporting the Irregularities on the basis of art. 6 para. 1 letter f) of the European Parliament Regulation and the Council (UE) 2016/679 on the protection of natural persons and on the free flow of such data as well as on repealing the directive 95/46/WE (general regulation on personal data protection) („The Regulation”);
1.3. The personal data of The Reporting Person will be stored for the period of the proceedings on the Irregularities, not longer than 6 months; In case of commencing the proceedings conducted on the basis of existing laws, the data is stored until valid completion of legal proceedings and for the period of the ensuing 12 months;
1.4. The Reporting Person is entitled to demand from the Administrator the access to the personal data concerning the Reporting Person, their correction, removal or restriction on the processing;
1.5. The Reporting Person is entitled to file a complaint to the Chairperson of Urząd Ochrony Danych Osobowych (UODO, the Personal Data Protection Office);
1.6. The personal data of the Reporting Person will not be processed in the automated manner;
1.7. On the protection of the personal data with the Administrator, on may get in touch with Inspektor Ochrony Danych Osobowych (Personal Data Protection Inspector) under the email address: iod@krakchemia.pl.
